You may submit your medication refill  request by using one of the methods listed below:

• Call your Pharmacy to confirm that there are no more refills available.
• Contact the Clinic with the below information or schedule an appointment

Please have the following information available:

• Your Name
• Date of Birth
• Name of Medication
• Dosage of Medication e.g. mg or units
• How often Medication is Taken
• Name of your Pharmacy
• Contact phone number in case we have questions regarding your refill
